What are the common problems that cause a PCB (printed circuit board) to fail despite having the correct circuit schematic design?

I checked the schematic carefully, without any problems, but why does the pcb board fail?

There could be numerous problems with PCB as follows:

  1. PCB footprints were made incorrectly by board layout person
  2. Too tight of clearance between traces and trace width. Board house simply couldn't manufacture board because of this.
  3. Trace width too small too handle current, so trace acts like a fuse and burns open.
  4. Solder mask or silkscreen on pad.
  5. Not enough annular ring around vias or could be manufacturing issue with hole alignment in via.
  6. Via exposed under part with a ground pad causing short.

The best way to avoid PCB issues is to download and install DRC rules check into your PCB layout software from board house and run it on your board. Resolve any issues you find first.
